Congratulations on your acceptance to Biola University! We hope you enjoy excellent health during your time on campus, and when you need credible health information or caring medical attention, we at the Biola Health Center will be here for you!
As you get ready to come to campus, your to-do list may seem endless. You'll need to take care of some health-related tasks before you arrive. Don't worry, we are here to help if you need us, and please contact us if you have any questions.
Important Note: As part of your transition to Biola and the La Mirada area, you should know your HMO or PPO service area and determine how you will access health care in the event of an emergency or need for specialized care. Communicate with your insurance company to arrange for a provider in the La Mirada area.
Required Forms
Biola University requires health paperwork for all new on-campus students, both undergraduate and graduate. For students starting in the fall, these forms are due by July 31. For students starting in the spring, these forms are due December 15. Please contact our office with any questions at (562) 903-4841 or via email at health.center@biola.edu.
The following forms must be submitted online by logging into your Patient Portal:
- Health History Form
- Meningitis Advisory
- Notice of Privacy Practices
- Tuberculosis (TB) Screening Questionnaire
- Immunization Records*
Important Note to Minors:
In addition to filling out the required electronic health forms, students who are minors must print, fill out and submit the following forms to the Health Center. Forms can be submitted by email to health.center@biola.edu, by mail to attn: Biola Student Health Center, 13800 Biola Ave, La Mirada, CA 90639, by fax at (562) 906-4512 or in person.
Important Note to Athletes:
The Athletic Department requires new student athletes to complete the Athletic Physical Forms (along with the required health forms) and are submitted directly to athletics. The athlete must also provide a copy of the Physical Exam to the Health Center.
